Consultation Forms

To provide holistic therapies, and give you the best and safest treatment possible, we will complete a consultation form before the treatment commences.

Consultation forms are securely stored and can only be accessed by Rose Dennigan. Consultation forms will be held for 6 years from the last use of my services.

The information recorded on the consultation form is name, email address, DOB, information regarding health and lifestyle, treatment dates and contraindications.
